Honouring Identity, Nurturing Connection
Living with Dementia doesn’t mean giving up quality of life. After an initial adjustment period, we focus on preserving independence, emotional well-being, and daily enjoyment. With Trinity HealthCare Plus, both clients and families are supported through a tailored, dignified approach that values each person’s story, routines, and individuality.
Continuity & Normalcy Matter
Consistent care teams help build trust, reduce anxiety, and enhance communication.
Maintaining daily routines and familiar surroundings supports cognitive clarity and lowers confusion.
Enabling clients to keep doing familiar tasks, like making tea or dressing themselves, preserves autonomy and can reduce hospitalisation risks.
What We Offer
At Trinity HealthCare Plus, we stand firmly against the myth that dementia diminishes a person’s capacity for a rich, fulfilling life. A diagnosis changes the journey, not the destination.
Personalised Routine Care
Support with morning and evening routines, mealtimes, medication, hygiene, and meaningful activities, delivered consistently to reinforce daily structure.
Cognitive & Sensory Engagement
Tailored experiences: memory prompts, sensory aids, simple puzzles, music, and art therapy to boost mood and brain health.
Reminiscence & Life-story Integration
Using life-story books, photos, and familiar sounds to evoke memories and reinforce identity.
Social & Physical Support
Gentle exercise, garden visits, community engagement, structured to match abilities and preferences.
Safety & Wandering Prevention
Risk assessments, environmental adaptations, visual cues, and respectful supervision to reduce wandering and keep clients secure.
